SUPPORTED LIVING

Our Supported Living program offers assistance, care, and community integration to individuals with intellectual disabilities.

From licensed community homes to apartments with in-home care, we have a variety of residential settings for individuals to call home.

Supported Living allows us to provide choices, encourage independence, and offer appropriate assistance so individuals can take control of their lives. Anne Grady Supported Living options include services ranging from basic in-home aid to 24-hour supervision.

Funded by Medicaid Individual Options (IO) and other waivers, the program provides wide-ranging assistance. Clients and staff work together to develop Individual Service Plans (ISP) that fit each person's unique desires and needs.

Services offered include:

  • Grocery and mealtime assistance
  • Housekeeping and laundry
  • Medication administration
  • Medical appointments
  • Social activities and companionship
  • Personal care
  • Transportation
  • Representative payee for Social Security

Remote Monitoring

Anne Grady offers remote monitoring backup support under the Medicaid IO waiver program. Trained staff can monitor individuals living alone using live video and audio feeds, motion-sensing devices, web-based security systems, or other methods. Remote monitoring allows individuals to live with less direct supervision, but with ready help in emergency situations.
